The most important thing to know about clicks: on Ozempic pens, 37 clicks always equals a full dose — but what that full dose is depends on which pen you are holding. The red pen delivers 0.5mg at 37 clicks. The blue pen delivers 1mg at 37 clicks. The yellow pen delivers 2mg at 37 clicks. Same clicks, different drug concentrations. This is why using the wrong pen causes dosing errors.

The 0.25mg starter dose is not effective for weight loss — it is used to allow the body to adjust to the medication and reduce initial side effects. Most people begin seeing meaningful appetite suppression at 0.5mg. Each dose increase should be maintained for a minimum of 4 weeks before advancing, and only if tolerated. Not everyone needs to reach 2mg — some people achieve their treatment goals at 0.5mg or 1mg.
Mounjaro and Zepbound Dosage Chart
Tirzepatide is the active ingredient in both Mounjaro (approved for type 2 diabetes) and Zepbound (approved for weight management). Unlike semaglutide pens which require dialling a dose, the Mounjaro and Zepbound KwikPen is a single-dose pen — each pen contains one complete dose and is discarded after use.

Mounjaro and Zepbound titration schedule
Starter dose only — not effective for glycaemic control or weight loss at this level. Used to allow the body to adjust and minimise GI side effects. Do not advance early.
First therapeutic dose. Most people begin to feel significant appetite suppression at this level. Hold for minimum 4 weeks before advancing.
Many patients find 7.5mg to be their optimal maintenance dose — strong effect with manageable side effects. Some prescribers recommend staying here if goals are being met.
SURMOUNT-1 trial showed 10mg produced approximately 19.5% average body weight loss at 72 weeks. A common maintenance dose for people who tolerate higher doses well.
Intermediate high dose. Used when additional effect is needed and 10mg is well tolerated. Not always required — advance only if recommended by your prescriber.
Maximum approved dose. SURMOUNT-1 showed 15mg produced 20.9% average body weight loss at 72 weeks — the highest result ever recorded for a non-surgical weight loss intervention at time of publication. Not all patients need or tolerate this dose.

There is no validated direct dose equivalence between semaglutide and tirzepatide. Approximate comparisons circulate online but these are not supported by clinical trial data or regulatory guidance. When switching between these medications, the standard clinical approach is always to restart at the lowest dose of the new medication and titrate as tolerated.

Frequently Asked Questions
On the red label Ozempic pen (the 0.25mg/0.5mg starter pen), 0.25mg is approximately 18 to 19 clicks from zero. The dose counter window on this pen will display 0.25 at the correct position — you do not need to count clicks as the dose is visible in the window. Always use the dose window, not click counting, when it is available. On the yellow label 2mg pen, 0.25mg would be approximately 9 clicks — but Novo Nordisk does not recommend using a higher-dose pen to deliver a lower dose by click counting.
On the yellow label 2mg Ozempic pen, 1mg is approximately 37 clicks — the same number as a full dose on the blue label 1mg pen. However, the drug concentration in the 2mg pen is double that of the 1mg pen, so 37 clicks delivers 2mg, not 1mg. To get 1mg from a 2mg pen you would need only 18 to 19 clicks — but this is off-label, not validated by Novo Nordisk, and not recommended. Always use the pen prescribed for your dose.
The standard semaglutide titration for weight management: 0.25mg for weeks 1 to 4 (starter dose, not therapeutic), 0.5mg from week 5 for at least 4 weeks, 1mg from week 9 if additional effect is needed, and 2mg from week 13 if required. There is no direct validated dose equivalence between semaglutide and tirzepatide. When switching from Ozempic or Wegovy to Mounjaro or Zepbound, the standard clinical approach is to stop semaglutide and start tirzepatide at the lowest dose of 2.5mg after a recommended 5-week washout period. Always discuss medication switches with your prescribing clinician before making any changes.
Tirzepatide has a half-life of approximately 5 days. This means each weekly injection adds to the drug still circulating from the previous dose — rather than starting fresh each week. It takes 4 to 5 weeks to reach true steady-state plasma concentration at any given dose level. In the first few weeks at a new dose, plasma concentration is still building, which is why week 2 and week 3 often feel stronger than week 1. This is normal pharmacokinetics, not a problem with the medication.
